Open Sky is seeking compassionate individuals who are looking for a great opportunity to grow a rewarding and meaningful career in human services. The Navigator, under the direction of the Clinical Director and Operations Director, will provide In-Reach and Re-entry supports, assessment, education, support and safety planning for individuals enrolled in the Behavioral Health Supports Justice Involved Program. The Navigator serves as a point of contact for integrated care coordination and care transitions in collaboration with individuals enrolled, justice agencies, medical and behavioral health providers, pharmacies, various social service agencies and other agencies and supports as indicated.
Bachelor's degree in health-related field preferred. High School Diploma/GED and/or applicable experience in community-based behavioral health supports, substance use disorder services and community re-entry programs. Experience with justice involved population preferred. Relevant experience may be substituted for degree. Valid driver's license with a reliable and appropriately insured vehicle required. Experience with accessing local resources, navigating health systems. Must meet participating Correctional Institutions requirements for facility entry. Excellent communication, documentation, time management, and organizational skills strongly preferred.
