ESSENTIAL FUNCTIONS: Reports directly to the Director of Strategic Initiatives or designated supervisor. This position coordinates, provides and ensures clinically-informed intake, transition, linkage and aftercare services to young people who are transitioning to/from other agencies, internal programs or level of care. The position is focused on utilizing agency, community, and family resources to ensure that transitions occur in a timely manner and seamlessly through the system. Requires knowledge of trauma exposure, knowledge of the 5 S's of trauma: Safety, Self-Regulation/Soothing, Supportive, Strengths, Self-Care, and an understanding of and recognition of the signs of traumatic stress. Must be able to effectively use both verbal and physical intervention skills with individuals six (6) through twenty-one (21) years of age. Physical intervention skills require the ability to kneel and return to standing without assistance.
JOB RESPONSIBILITIES: Coordinates intake and admission process in collaboration with team from Residential and Programs Manages referrals to the Residential, programs through the email stream on the D-Net Responds to the stream within timeframes established by the position, programs and supervisor Works collaboratively with Residential, staff to reach census in programs Works collaboratively with our contractors to streamline the Intake Process Coordinates transition stabilization and support services for young people entering, transitioning through or leaving the Lawrence Hall system Facilitates clinical transition staffings, develops and implements action plans, and is responsible for the development of a formal transition plan for each young person Completes staffing reports and other DCFS required documents within specified time frames Collaborate with Therapists, Case Managers, intake team, and Site Coordinators/Treatment Team Coordinators to determine the needs of young people that are in transition Attend discharge meetings held by other agencies when the young person has been identified as a referral for Lawrence Hall Interprets agency's services to assigned young people and their families Attends Agency and other professional seminars and conferences to increase professional knowledge and skills When necessary, liaison to DCFS or other agency (courts, school districts, etc.), communicating goals and progress toward their accomplishment Maintains electronic calendar to provide accounting for daily activities and provides permissions for viewing to the appropriate individuals Attends all required staffings Performs the above tasks under close weekly support, supervision, training, and direction of supervisor Completes clinical documentation in accordance with Medicaid timelines and requirements Uses active and reflective listening skills and affirmations to demonstrate respect and empathy, avoiding negative and derogatory labels that foster disrespect Understand that behaviors, responses, attitudes, and emotions are a collection of survival skills often developed in response to traumatic experiences Maintain confidentiality of employees, volunteers, and program participants to provide a safe environment Attend at least two trainings related to being trauma informed in a twelve-month period Ability to work effectively with program participants and family members, employees and vendors in a culturally diverse environment to support healing and recovery Participates in the development and implementation of continuous quality improvement activities as assigned Maintains flexibility to adjust work schedules according to the demands of the program Other duties as assigned QUALIFICATIONS: Bachelor's degree in Social Work, Psychology or related Human Service field Computer proficiency using Microsoft Word, Microsoft Outlook and Microsoft Excel Knowledge of childhood trauma and prior experience working in settings with youth and families that have been exposed to trauma or experiencing traumatic stress preferred Knowledge of youth development, adolescent issues, positive youth development/strength-based approach preferred Personal vehicle or access to vehicle and proof of insurance Valid Illinois Driver's license Ability to pass background clearance and drug screen This position meets the FLSA requirements for Exempt Job descriptions are not intended, and should not be construed, to be all-inclusive lists of all responsibilities, skills, efforts or working conditions associated with a job.
