Description Summary:
Key member of the Transplant Center's Multidisciplinary Team. The Transplant Substance Use Navigator will be responsible for working with individuals identified to have a substance use disorder who are in the process of pursuing a solid organ transplant. This role will facilitate various areas of care related to the substance use disorder including but not limited to treatment referrals, tracking treatment progress, monitoring toxicology results, etc. This role will be a member of the Transplant Social Work team but will work alongside various disciplines.
Work Schedule: Full Time Days
Responsibilities: Participates in case reviews to evaluate care management and progress toward goals. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Periodically administers and interprets tests and measures of psychosocial functioning for the purpose of diagnosing mental, emotional, behavioral, addictive and developmental disorders and disabilities, implementing appropriate assessment-based treatment plan. Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action. Requirements: Minimum Required Education: Master's degree in Social Work. Required Licensure/Certification: State licensure as a Licensed Clinical Social Worker (LCSW). Minimum Experience: None Required. Are you a RN New Grad? The pay range for this position is: $31.61 - $47.41 / hour. Pay is dependent on applicant's relevant experience.
