Social Worker - Skilled Nursing/Short-Term Rehab

Position Summary: Evaluate and assess applicants/residents and/or family members for medical, emotional, social, financial and cultural background for placement and discharge in accordance with the policies of the Social Services Department. Responsible for promoting teamwork with all members of the healthcare team. Performs all duties in a manner consistent with St. Luke's mission and values.  
Education, Experience, & Licensing Requirements: Education: Master's Degree in Social Work
