• Complete psychosocial assessments and document in EMR• Identify care planning goals and develop interventions• Participate in interdisciplinary team meetings• Provide education on funeral planning and advanced directives• Conduct visits with patients and caregivers• Assist with bereavement support• Participate in on-call rotation• Qualifications• Hold a MSW or BSW degree• Social work licensure in certain states• 1 year of social work experience in healthcare setting• Excellent communication and interpersonal skills
