Responsibilities
• Assess the psychosocial status of the patient/client• Deliver care based on the physician's plan of care• Report on the condition of patient/client to appropriate supervisor and staff members• Provide social work services including counseling and crisis intervention• Develop the plan of care in consultation with the physician and team members• Perform specialized assessment based on psychological, biological, physical, and social sciences• Document treatment and interventions provided, as well as patient's response• Assist other team members in understanding social and emotional factors related to health problems• Coordinate care for assigned patients/clients and utilize community resources• Communicate with multidisciplinary healthcare team and participate in quality improvement measures
Requirements
• Master of Social Work (MSW) degree preferred• State license to practice where applicable• 12 months of experience within the last 5 years• Ability to communicate with all levels of the workforce• Ability to perform multiple functions, tasks, and changes• Ability to read and interpret technical instructions• Ability to observe and assess patients• Ability to travel locally• Valid driver's license and auto liability insurance• Meets applicable health requirements and proof of Covid vaccine
