What is The Transition Academy?
The Transition Academy (TTA) is a nonprofit organization founded in August 2019 to improve postsecondary outcomes for high school special education students in Kansas City Public Schools and charter schools. Our mission is to make economic inclusion a reality for youth with disabilities, especially for young people of color who have historically been failed by systems.

TTA is an interdisciplinary team of professionals. Every member of the team (including the CEO) works together on real world learning activities for students. Real World Learning Activities include bringing employers to classrooms to work with students and taking students to job sites to participate in hands-on career exploration activities.

Why Social Workers Are Needed for Real World Learning
We aim to support high school students who learn differently to achieve overall social, emotional, and mental health as they prepare for post-high school employment. These characteristics not only facilitate a smoother transition to the workforce but also contribute to long-term career success and personal well-being.

Youth with disabilities have been in school most of their lives with teachers and predictable school day activities. Once they leave school, they're on their own. Employers don't play the same role as teachers. Students must initiate employment, learn skills, learn new work hours, work with supervisors and coworkers. Developing strong social and emotional skills helps youth manage stress and pressure effectively, which is vital when transitioning to the workforce.

What Are the Social Services Manager's Duties and Responsibilities?
We developed a Social Services Team during the ******** school year to provide trauma-informed mental health and social emotional support to help these youth achieve postsecondary success.

The Social Services Manager will lead the Social Services Team (full-time and part-time associates) in facilitating assessments, weekly in-classroom sessions, social emotional assessments, health care benefits guidance, parent support/contact.

Supervisory Duties and Responsibilities include:
Conduct youth assessments and evaluations with program participants/legal guardians during the intake process.
Use student, school and family assessment results to identify needs that impact postsecondary success.
Provide short-term individual and group interventions to students who are experiencing social/emotional/behavioral problems that interfere with their high school and postsecondary success.
Develop processes for coordinated referral, services, and follow-up to high school-aged youth and their families for evidence-based practices.
Forge partnerships and trust with other district departments, principals, school teams, and community stakeholders.
Develop a comprehensive plan of community-based mental health services, including referrals to crisis counseling, that utilize evidence-based practices and are developmentally appropriate school and community based mental health practices and services.
Maintain accurate case records and documentation.
Complete additional duties as assigned by the supervisor.

Desired Qualifications:
Master's Degree from an accredited college or university in social work, psychology, sociology, human services or other related field.
Prior supervisory experience and/or people development experience.
Three (3) years of direct work experience with socio-economically challenged youth in culturally and ethnically diverse urban settings.
Three (3) years of case management experience.
Three (3) years of individual and small group intervention.
Previous experience working with school systems and local, state or federal agencies.
Ability to plan, organize, and work collaboratively with partner organizations.
Ability to work flexible hours, including some evenings and weekends, and may include home visits.

Salary Range

$60,000-$65,000
