There is follow-up to verify that all aspects of each care plan are being properly implemented. Information about each resident's health issues is clearly communicated to relevant departments and government agencies. Discharge planning sessions are held in which information is provided to residents and their families about available programs and resources. Maryville is kept abreast of changes in current federal and state regulations that may affect Social Services Department policies and procedures. Department staff do their part to maintain Maryville's atmosphere of peace and healing. Requirements: Bachelor's Degree in Social Work (Master's Degree strongly preferred). 3 – 5 years of experience in social services in a long-term care or similar health care setting. Registration with the Academy of Certified Social Workers or membership in the National Association of Social Workers.  Knowledgeable of the rules, regulations and guidelines that govern long-term and resident care facilities Understanding of Medicare, Medicaid, and private insurance programs. The proven ability to communicate effectively and empathically with Residents, their family members, and staff at every level of the organization. A strong commitment to maintaining the confidentiality of all resident care information in accordance with HIPAA guidelines. Intermediate computer skills. Expert knowledge of both the MDS process and Medicare/Medicaid eligibility. The ability to pass the background check required of all Maryville employees. More about Maryville
