Maryville is seeking a full-time Social Services Coordinator to join its well-respected team of dedicated health-care professionals. Responsibilities include: Ensuring that the medically related emotional and social needs of the resident are met and maintained. Developing assessments and written plans of care to identify the needs of each resident. Assessing the resident's psychosocial and emotional needs. Effectively communicating information concerning a resident's condition with other departments and government agencies. Participating in resident care planning conferences. Participating in the resident discharge planning, providing information to residents and families of programs available. Serving as a liaison with community agencies to ensure needed ancillary services are available and provided within requested timeframes. Making referrals and documenting outcomes. Qualifications: Bachelor's degree or equivalent experience is required; focused in Social Work or Healthcare administration strongly desired. Recent experience in a Social Services role in a nursing home/hospital setting or some other Social Worker role. Ability to work in a fast-paced work environment. Excellent communication and customer services skills. Strong computer skills required due to this facility working solely in an Electronic Medical Records environment.
