We are accepting applications for a Social Services Assistant who is available to work:
Part Time (less than 24 hours per week) Schedule to be discussed Starting wage between $15.00/hour - $16.00/hour (depending on experience)
Social Services Assistant: Support the rehab unit and assist with admissions and discharges. Assists and advocates for residents and their families while at our facility. Collaborates with the Social Workers and other facility departments as well as community resources. Assists residents in exercising and protecting their rights. Assists in creating individualized care plans for each resident. Assists residents and their families with resources pertaining to discharge planning and post discharge planning. Completion of timely and accurate case related documentation and assessments. Oversight of coordinating, managing, documentation and scheduling of ancillary services. Assists and participates in resident council and neighborhood meetings. Assists in scheduling Care Conferences with residents, family members, and staff. Assists staff in managing social and emotional factors and crisis intervention. Assist with audits. Assists Social Services department with workloads as needed. All other duties as assigned including and not limited to Administrative Assistant and Social Services duties. Follows and adheres to facility policies and procedures implementing HIPPA requirements. Assists in interdisciplinary meetings. Cooperates with expectations related to being a mandated reporter of abuse and/or neglect. Requirements Associates degree in the human services field with experience in health care preferred Experience with the geriatric population and experience working with people with Alzheimer's disease and related memory disorders preferred.
