Job summary
The primary purpose of this position is to provide skilled nursing care to residentsUnder the medical direction of the residents' attending physicianAnd within the scope of nursing practice for the state

Responsibilities
• Provide oversight of Certified Nursing Assistants (CNAs) and licensed nurses• Consult and coordinate with the interdisciplinary team (IDT) and healthcare professionals• Assess, plan, implement and evaluate individualized resident care plans• Complete initial baseline and periodic comprehensive assessments and care plans• Participate in completing sections of the Minimum Data Set (MDS) assessment• Facilitate physician rounds and document physician visits with residents• Initiate requests for consultations or referrals• Report any suspicion of a crime committed to a resident in the facility• Perform administrative duties by completing medical forms, reports, evaluations, charting• Participate in quality committees and attend various other committees of the facility
Requirements
• Must possess a nursing degree from an accredited college or university• Must have at least 2 years of experience as an LPN/RN• Must possess a current unencumbered active license to practice as an LPN/RN• Must be able to read, write, speak and understand the English language• Must demonstrate knowledge and skills necessary to provide age-appropriate care• Must possess leadership and supervisory ability• Must possess the ability to plan, organize, and interpret programs and policies• Must have patience, tact, a cheerful disposition, and enthusiasm• Must be able to relate information concerning a resident's condition• Must meet general health requirements set forth by the facility
