Description: Department: and nbsp;Observation Unit Shift timing: and nbsp;19:0007:30 and nbsp;Hours Monday to friday (3 Days/12 Hours) Per Week: 36 Hours. Roles and amp; Responsibilities Implement and monitor patient care plans. Monitor record and communicate patient condition as appropriate. Serve as a primary coordinator of all disciplines for wellcoordinated patient care. Note and carry out physician and nursing orders. Assess and coordinate patients discharge planning needs with members of the healthcare team.
