Job Summary: The registered nurse plans, organizes, and directs hospice care, with an emphasis on community health education and experience. As an RN Hospice, you will utilize your nursing skills to provide professional care to patients and their families within their homes and communities. Your responsibilities will include completing comprehensive assessments, developing care plans, administering medications and treatments, coordinating patient care, and providing emotional support to patients and their families.
Duties & Responsibilities: Patient Care: Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs Provide professional nursing care utilizing the nursing process Develop care plans based on nursing diagnosis and incorporate palliative nursing actions Administer medications and treatments as prescribed by the physician Assist patients with activities of daily living and facilitate self-sufficiency and comfort care Act as a case manager and coordinate patient care Communication: Maintain accurate and relevant clinical notes regarding patient's condition and care Communicate with physicians and community health-related persons to coordinate the care plan Teach patients and families self-care techniques and provide health counseling Provide a safe environment for patients Work in cooperation with the interdisciplinary group to meet emotional needs of patients and families Additional Duties: Participate in on-call duties as defined by the on-call policy Supervise ancillary personnel and delegate responsibilities when required Maintain and upgrade professional knowledge and practice skills through continuing education Actively participate in quality assessment performance improvement teams and activities Job Requirements: Graduate of National League for Nursing accredited school of nursing Minimum of two years of experience, with at least one year in public health, home care, or hospice nursing preferred Registered nurse with current licensure to practice professional nursing in the state Demonstrates excellent observation, problem-solving, and communication skills Ability to organize and prioritize workload independently Maintains a current CPR certification Must be a licensed driver with an insured automobile in good working order Self-directed and able to work with minimal supervision Perks and Benefits: Flexible schedule Paid time off Health insurance Dental insurance Vision insurance 401k matching Employee discount Mileage reimbursement Location: Kingman, AZ
