Responsibilities
• Coordinate patient care with interdisciplinary team• Administer skilled nursing care in patient's place of residence• Case management and clinical direction• Develop and document patient plan of care• Provide care utilizing infection control measures• Assure continuity of quality patient care• Monitor compliance with third-party payer requirements• Demonstrate professional growth and competency• Promote agency philosophy and policies• Supervise and provide direction to home health aide and LPN
Requirements
• Education: Graduate of an accredited nursing program• Licensure: Current State license as a Registered Nurse, current Driver's License• Experience: Two years of experience in a clinical care setting, home health or hospice preferred• Skills: Nursing skills, good interpersonal skills• Proof of CPR, Hepatitis consent/declination, and Covid Vaccination or eligibility for a religious/medical exemption• Transportation: Reliable transportation, valid and current auto liability insurance
