Join our team as a Clinical Care Coordinator, where you'll play a vital role in managing the care of our members with complex medical and behavioral needs. This full-time role offers flexibility and the opportunity to work from a home-based office, with occasional field visits throughout the Northeast region of New Mexico.
Key Responsibilities:
Engage members through face-to-face or telephonic interactions to conduct thorough needs assessments encompassing medical, behavioral, functional, cultural, and socioeconomic aspects.
Develop and execute personalized care plans aimed at addressing chronic health conditions, wellness promotion, social determinants of health, medication management, and member safety in accordance with evidence-based guidelines.
Collaborate closely with internal care teams, providers, and community resources to implement and monitor care plans effectively.
Provide education and coaching to empower members in self-managing their care needs and making lifestyle changes conducive to better health outcomes.
Facilitate proactive discharge planning and oversee care transitions following ER visits, inpatient stays, or Skilled Nursing Facility admissions.
Advocate for members and their families to ensure their needs and preferences are fully supported by the healthcare team.
Requirements:
Current, unrestricted licensure as a Registered Nurse.
Minimum 3 years of clinical experience, with at least 2 years in healthcare.
Proficiency in MS Office applications (Word, Excel, Outlook).
Reliable transportation and willingness to travel within the assigned territory (up to 50% of the time).
Residency in New Mexico.
Preferred Qualifications:
Master's degree or higher in a clinical field.
Certification as a Commission for Case Manager (CCM).
Experience in community case management for individuals with complex needs.
Background in managed care.
Familiarity with team-based care models.
