What you will work: .8 FTE = 64 hours every 2 week pay period Monday - Friday, Day time hours What you will do: Work with healthcare professionals and community liaisons to identify potential patients through screening, consultation, and referrals Interact with patients on a daily basis prior to admission to support them to complete the appropriate paperwork. What you need: Bachelor's degree in Psychology, Social Work or Addiction Studies 1 year experience working in human services field or health care setting Valid Driver's License (DL) Current Infant-Adult Basic Life Support for Health Care Providers (with hands on learning) from one of the following programs; American Heart Association or American Red Cross Job Description: The Residential Admission Specialist works under the direction and supervision of the Clinical Manager of Gundersen Health System - Unity House. The responsibilities are to oversee and assist in the continual development of the admission process for Unity House in order to meet the evolving needs of the patient population and funding sources. The Residential Admissions Specialist assesses and prioritizes referrals both internal and external and supports Unity House to ensure standards are being meet for admission criteria and quality measures. The Specialist coordinates work effectively in a professional and respectful manner to meet service objectives and maintains knowledge of program and process updates.
MAJOR RESPONSIBILITIES: 1. Acts as admissions specialist between referral sources and Unity House. Works with healthcare professionals and community liaisons to identify potential patients through screening, consultation, and referrals. When assessing referrals, utilizes critical thinking skills in gathering patient data, meeting the immediate needs of patients and families.
2. Interacts with patients on a daily basis prior to admission to support them to complete the appropriate steps regarding addiction assessments and physical history.
Utilizes knowledge and resources to assist patients with identifying funding sources available for their treatment. Uses critical and creative thinking to create successful complete financial assistance plans for patients.
3. Works interactively with County Case Workers to obtain a Service Plan for the patient.
4. Maintains a strong knowledge of the Legal System and is responsible for ensuring that referrals have any legal incarceration requirements resolved prior to admission.
5.Triages and screens patients upon admission for suicidal behavior and drug impairment and inspects belongings for any drug paraphernalia.
6.Completes admission process with the patient to ensure appropriate documents and requirements are being met, while working collaboratively with the Unity House team.
7. Attends weekly multidisciplinary Unity staffing and any other necessary staffing or meetings to discuss potential referrals and pertinent patient information.
8. Maintains professional working relationships with department staff members, Gundersen Health System departments, community agencies, funding sources, and other individuals as appropriate.
9. Shows a commitment to excellent customer service by providing prompt and professional assistance to internal and external customers through verbal, written and electronic communication
10.Collects and analyzes statistical data to determine efficiency of admission process, monitors quality measures, develops reports that may identify gaps in the admissions process.
11. As assigned, assists in community partnerships for the development of local and statewide resources.
12. Maintains a high level of knowledge of billing regulations and requirements for residential treatment for all government payors.
13. Completes required Medicaid documents to assure funding for treatment.
14. Completes follow-up phone calls for discharged patients in a timely manner to meet compliance measures.
15.As assigned, assists in the development of brochures, and other promotional materials.
16.Facilitates and provides transportation as necessary for patients when admitting to Unity House.
17. Performs other job-related responsibilities as requested.
Education and Learning: REQUIRED
Bachelor's degree in Psychology or Social Work or Addiction Studies
Work Experience: REQUIRED
1 year experience working in human services field or health care setting
DESIRED
1 year experience working in behavioral health or addiction setting
License and Certifications: REQUIRED
Valid Driver's License (DL)
Current Infant-Adult Basic Life Support for Health Care Providers (with hands on learning) from one of the following programs; American Heart Association or American Red Cross.
Age Specific Population Served: Adult (18 - 64 years)
OSHA Category: Category I - All Employees in this job title have a reasonably anticipated risk of occupational exposure to blood and/or other potentially infectious materials.
Environmental Conditions: Exposed to infectious diseases.
