Position Summary: This position is responsible for general Registered Nurse (RN) clinic duties including triage of phone and walk-in patients, rooming patients, and providing support to the providers as requested, as well as face-to-face office visits for patients with chronic or acute conditions as defined by the White Bird Clinic Nursing Protocols in consultation with the supervising provider . Primary Responsibilities: Perform in person triage function. Specifically identify patients who need immediate referral to local Emergency Departments with patient report phoned to receiving facility. Identify and request registration of patients to be seen by physicians. Identify and request registration of patients to be seen under Nursing Acute Protocol visits. Provide advice and written resource information to those patients who cannot be seen. Perform phone triage function in the moment or within 4-hours of patient call and coordinate with the Front Office to schedule appointment, advise to seek a higher level of care such as Urgent Care or Emergency Department or general advice within scope of practice using approved triage manual. All communication must be documented in the electronic medical record. Possess ability and willingness to work in an EHR (electronic) environment and to meet or exceed the documentation requirements to meet national standards for Clinical Quality Measures and other clinical quality benchmarks required by all our grant funders and clinic-accepted initiatives. Room patients for and provide support to providers as requested, this can include individualized wound care, procedural assistance, room chaperone point of care testing, drawing labs and preparing for Lab pick-up, dispensing medications from dispensary, as well as other duties assisting other Medical Clinic employees to keep the clinic flow fluid and safe. Vaccine management through the Alert Immunization Information System as well as 340b medication management will be expected within 6 months of hire. Provide nurse visits by following standing orders and nursing protocols to order labs, referrals, medications, provide education and support patient goals to improve health. This includes displaying knowledge of Chronic Disease processes including but not limited to Diabetes, Hypertension, Asthma, COPD, Hyperlipidemia, and the management of warfarin. Conduct comprehensive assessment of patients' physical, mental, and psychosocial needs, as written, in protocols and standing orders to develop care plans to prevent disease exacerbation, improve outcomes, increase patient engagement in self-care, decrease risk status, and minimize hospital and ED utilization, doing so all within scope of nursing practice and in coordination with supervising provider. Coordinate care with external agencies as needed, requested by supervising provider. Document all patient communication and care provided in EMR. Other duties as assigned. Required Qualifications: Current Oregon RN license. Minimum of 5 years of nursing experience that includes Med/Surg, Urgent Care, or Emergency Department. Covid vaccination - primary series must be completed. RN with bachelor's degree or equivalent experience Ability to work with a diverse population and in a broad range of situations, while maintaining a professional and courteous attitude. Ability to work as part of a highly functional and collaborative team. Willingness to participate in continuing education. Experience in the delivery of primary care. Ability to work effectively with minimal supervision. Strong computer skills. The ability to send and receive email, write documents in MS Word, look up information on the internet, and use an electronic EHR system. Maintain current AHA-BLS, ACLS (ACLS to be completed within 6-months of hire) and complete annual White Bird Clinic training to include CLIA point-of-care testing, and respiratory mask fitting. Receive approval of the Medical Director showing complete knowledge of all the Clinic's Acute and Chronic Care Nursing Protocols within 3 months of hire date. Complete NBT. This position requires successful passage of a State mandated background check. Preferred Qualifications: Bilingual in English and Spanish Experience working in a healthcare setting providing services to a nontraditional client base, i.e., the unhoused, non-native English speakers or other at-risk populations. Compensation Details: This is non-exempt hourly position with target starting base compensation ranging between $35/hour to $40/hour paid on a bi-weekly basis.
