Responsibilities
• Manage a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community.• Serve as a patient advocate.• Communicate effectively with patients, families, and interdisciplinary healthcare team members.• Assess, plan, implement and evaluate patient care plans.• Coordinate and collaborate with other members of the healthcare team.• Document patient information accurately and timely.• Provide education and support to patients and their families.• Adhere to all nursing standards, policies, and procedures.• Maintain confidentiality and comply with HIPAA regulations.• Participate in quality improvement initiatives.
Requirements
• Valid Nursing License (RN) in the State of New Hampshire.• Nursing experience required.• Recent wound care experience preferred but is not required.• Current CPR BLS Certification required.• Reliable transportation.
