Responsibilities
• Assess assigned case load of behavioral health care patients and families• Assume primary responsibility for caseload and develop plan of treatment• Evaluate patient needs and incorporate data into plan of care• Make initial evaluation and re-evaluate nursing needs during each visit• Communicate significant findings and changes in health condition• Revise plan of care in response to identified patient care issues• Counsel, instruct, and include patient, facility, and family in plan of care• Teach behavioral health philosophy and pain management to staff in alternate care settings• Supervise and document delivery of care by behavioral health aide and LPN• Inform supervisor of unusual or problematic nursing facility issues• Provide support at time of death and perform bereavement assessment
Requirements
• Current, unrestricted state RN license• 1+ years of experience in a clinical care setting• Able and willing to travel within branch/office coverage area• Must have a dependable vehicle, valid driver's license, and current auto insurance• Must be able and willing to travel 50%• Associates Degree or Bachelor's Degree in Nursing is preferred• Previous Home Health or Hospice Experience is preferred• Previous experience with Homecare HomeBase is preferred
