We are currently seeking a dedicated and skilled Registered Nurse (RN) to provide in-home nursing care to our diverse patient population. Key Responsibilities: Conduct initial patient assessments and develop individualized care plans. Administer medication and monitor patient health status. Provide wound care, IV therapy, and other skilled nursing services. Educate patients and their families on disease processes, treatment, and home health care procedures. Coordinate with physicians and other healthcare professionals to ensure optimal patient outcomes. Maintain accurate and detailed patient medical records. Qualifications: Current RN license Minimum of 1 year of nursing experience (home health experience preferred) Proficiency in clinical skills with the ability to work independently. Excellent communication and organizational skills. Valid driver's license and reliable transportation.
