Responsibilities
• Ensures activities align with Elara Caring's vision• Maintains patient and staff privacy and confidentiality• Assesses patient's needs and develops a plan of treatment• Makes initial evaluation and re-evaluates nursing needs• Communicates significant findings to the Clinical Supervisor• Revises the plan of care as needed
Requirements
• Current, unrestricted state RN license• 1+ year experience in a clinical care setting• Able and willing to travel within coverage area• Must have a dependable vehicle, valid driver's license, and current auto insurance• Associates Degree or Bachelor's Degree in Nursing preferred• Previous Home Health or Hospice Experience preferred• Previous experience with HomeCare HomeBase preferred
