They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs. This role is crucial in ensuring patients receive timely and effective care tailored to their specific behavioral health needs.Job Duties/Responsibilities: + Perform all aspects of the hospital's assessment and referral process.
+ Service coordination by matching patients with appropriate mental health services and providers within UHS , considering factors such as specialty, location, and systems complexities. and insurance coverage.
+ Responsible for providing and assisting patients with resource navigation, assisting patients in understanding their treatment options and guiding them through the healthcare

Qualifications EDUCATION Master's degree preferred in Social Work, Psychology, Counseling, Behavioral Health or related field. LICENSURE / CERTIFICATION Licensed LPN Current CPR , Vaccines, if required by a customer hospital. ("Customer hospital" means the hospital to which BHC is contracted to provide mobile assessment services.)Must have a valid driver's license.
