Referral Coordinator Qualifications: Minimum of 1 year of hospice experience required. Experience in a medical office or healthcare setting is required. Excellent time management skills, detail-oriented tendencies. Must have computer training and Windows-based applications experience required. Must have reliable personal transportation, a valid driver's license, and the ability to safely and personally operate a vehicle. Strong understanding of electronic clinical record utilization. Responsible for maintaining complete confidentiality of patients' records and all other confidential information that may be received during the course of patient care, including financial information and personnel records. Ability to assign a patient to the appropriate team in coordination with the required team leader(s) after admission. Receive and promptly process referrals and/or inquires for services provided by hospice. Schedule with patient /family, a hospice representative to provide information on hospice care and determine the need for service. Extensive knowledge of hospice care and services.
