Seeking part-time physician to provide clinical collaboration for advanced practice providersCustomizing care that is preventive, proactive and includes all members of the care teamCare Model delivers customized care with reduced ER visits and hospitalization

Responsibilities
• Provide phone consultation and collaboration with 5-6 advanced practice providers• Provide in-person consultative care approximately 2 days per week• Respond to consultative requests within 2 hours• Participate in taking "call" from time to time• Oversee advanced practice providers• Maintain effective communication between physician and advanced practice provider(s)• Be connected to patient care• Graduate of an accredited residency training program• Board Certified or board prepared with the American Board of Medical Specialties or AOA• Licensed to practice within the state of Wisconsin
Requirements
• Ability to maintain DEA certification for the State of Wisconsin• Proven ability to communicate effectively with strong verbal skills• Excellent interpersonal and customer service skills
