Responsibilities
• Part of an integrative primary care team• Deliver primary care for patients in the community• Collaborate with embedded APP, behavioral health provider, clinical pharmacist, and case management team• Work in a clinic with 13 exam rooms and on-site physical therapy and diagnostic imaging• Serve as a medical hub for families and aging adults
Requirements
• Full-time, flexible between 0.75 - 1.0 FTE 4 days/week schedule (M-F)• Board-certified or board-eligible Family Medicine or Internal Medicine• 2-year guaranteed salary (with higher starting base for experienced physicians)• Generous quality incentives, sign-on bonus, and relocation assistance available• 2024 residents encouraged to apply
