Physical Therapist

The West Virginia Office of Shared Administration, Department of Health Facilities, Lakin Hospital, is inviting applications to establish an open-end contract for physical therapy services.
Qualifications:
Registered physical therapist in the State of West Virginia.
Mandatory Requirements:
Provide in-house physical therapy services for the residents of Lakin Hospital.
Complete evaluations for all residents and new admissions.
Be available for consultations upon request.
Document each resident visit and maintain medical records as per hospital policies.
Reassess residents and modify treatment plans in coordination with the attending physician and treatment team members.
Recommend and develop treatment/care plans for residents on restorative treatment plans.
Train Restorative Therapy personnel as per facility requirements and residents' needs.
Participate in appropriate committees and provide indirect supervision to restorative therapy staff.
Recommend and evaluate adaptive equipment and seating devices.
Benefits:
