Organization
The Parent-Child Assistance Program (PCAP-1) is a one-year case management intervention serving pregnant and parenting mothers with at-risk alcohol or drug use during pregnancy and their families. As part of a three-year, grant-funded research project, the program is hiring staff to enroll and serve clients in Enid, Oklahoma. The Case Manager position is responsible for assistance with client enrollment, community outreach, home visitation, case management, and advocacy services. Job Responsibilities Responsible for the case management process including admission, assessment, reassessment, service planning, monitoring and discharge of consumers. Provides support to cope with chronic or acute illnesses or conditions. Advises care givers, provides patient education and counseling, and makes referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare. Collects in-depth information about a client's situation and functioning through client interview and other relevant sources to identify individual needs. Develops a comprehensive case management plan that will address identified needs. Monitors, and evaluates treatment plans and progress. Facilitates interdisciplinary approaches. Monitors staff performance. Develops and implements action-oriented and time specific case management plans by determining and documenting specific objectives, goals, and actions designed to meet the client's health and human service needs identified through the assessment process. Organizes, secures, integrates, modifies, and documents the resources necessary to accomplish the goals set forth in the case management plan. Gathers information and documentation from all relevant sources regarding the case management plan's activities and/or services to determine the plan's effectiveness in reaching desired outcomes and goals. Modifies or changes the case management plan, as needed. Measures and analyzes the interventions to determine the outcomes of case management involvement, e.g. clinical, financial, variance, quality, quality of life, client satisfaction. Performs other duties as needed to successfully fulfill the function of the position. PCAP-1 duties include:  Satisfactorily complete intensive training on the PCAP model and other topics.Conduct outreach to community providers to build program support and case referrals to PCAP-1. Identify high-quality services in the community to which to refer PCAP-1 clients to according to their needs.Create a database of local resources available to clients within the program.Perform case management and home visiting activities according to PCAP-1 policies and procedures in a highly confidential and professional manner.Manage caseload of 20-23 clients/families and conduct home visits with each client approximately twice per month according to the PCAP-1 model.Assist clients in setting their own goals and steps to achieve them; monitor and document client progress.Engage and connect clients with community agencies that will help meet their needs and goals.Transport clients to important appointments in support of their goals, if needed. Assist the research staff in administering assessment instruments, according to PCAP-1 policies and procedures.Use referral and case management software and the research database to securely enter client information.Coordinate scheduling and attend case consultation meetings and appointments with community providers with whom clients are involved.Maintain regular contact and coordinate care with clients¿ networks of service providers.Provide housing support/case management services and documentation as required for housing subcontracts and working agreements.Attend and participate in weekly individual supervision with the Case Management Supervisor to discuss client needs, goals, and issues.Participate in weekly staff meetings with the local PCAP-1 team to discuss client and program progress.Provide outreach to locate missing clients and engage them with the program.Participate in required trainings.Obtain and maintain current CPR, Infant CPR, and First Aid Certification.Contribute to report writing, training, and presentations as needed.Perform other duties as assigned. **** The positions will be located in Enid, Oklahoma and surrounding area.***
Job Location
Required Education: Bachelor's Degree in Social Work, Psychology, Sociology, or other Health or Human Services related field. 12 months experience in home or community-based services, case management, or a related healthcare environment. Skills: Knowledge of federal and state laws particularly patient confidentiality.Knowledge of case management fundamentals and the technical and procedural aspects of case management.Must have a high level of organization skills.Ability to resolve issues, applying judgment, critical thinking, and problem-solving skills to determine a course of action.Ability to exercise discretion when making decisions and seeks managements advice to determine proper course of action while following established standards.Ability to handle stressful situations, different situations or circumstances, and complete projects in a timely manner.Ability to keep accurate and detailed records. Certifications:  None Departmental Preferences: 4 years of community-based experience working with at-risk populations.Experience working with clients/consumers/patients with substance use disorders, ideally perinatal individuals.Effective communication skills.Demonstrated ability to effectively engage with at-risk mothers and families.Ability to conduct interviews covering extremely sensitive and confidential issues.Familiarity with federal and state laws, particularly patient confidentiality.Knowledge of case management fundamentals and the technical and procedural aspects of case management.Strong organizational and time management skills.Ability to resolve issues, applying judgment, critical thinking, and problem-solving skills to determine a course of action.A willingness to exercise discretion when making decisions and seek management's advice to determine proper course of action while following established standards.Capability to handle stressful situations, challenging circumstances, and complete projects in a timely manner.Commitment to keep accurate and detailed records.A valid Driver's License.
