Compensation:
Hourly pay ranges from $26 to $28 for LPNs, and $29 to $31 for RNs.
Full-time employees are eligible for benefits after 90 days of employment.
Full-time employees with one year of service are eligible for a 401(k) with a company match on designated dates.
Highlights:
This position offers a contract assignment with the possibility of permanent employment based on attendance, performance, and business needs.
Fully remote opportunity, requiring residency in Oregon.
Shift timing: Monday through Friday, 8:00 am to 4:30 pm.
Responsibilities:
Conduct telephonic outreach on behalf of providers' offices to members identified as needing services to support quality initiatives and meet regulatory/contractual requirements.
Provide members with education about their care gaps in preparation for medical appointments.
Schedule doctor appointments for practitioners and assist members with wraparound services, such as arranging transportation and connecting them with community-based resources and affinity programs.
Perform chart review and abstraction, including closing applicable care gaps based on licensure.
Perform additional duties as assigned.
Requirements:
High school diploma or equivalent experience; Bachelor's degree preferred.
Clinical experience is required.
Prior experience in member outreach is preferred.
Mandatory attendance during the first 90 days.

Salary: $ 26.00 31.00 Per Hour
