Speech Language Pathology is medically prescribed and involves evaluation, creating and maintaining an individualized plan of care, treatment, re-evaluation, discharge, interprofessional communication and representation of service line.
In addition, the program coordinator will handle administrative functions as delegated by the Department Director.
Essential Functions: Complete comprehensive Evaluation/Re-evaluation: Assesses the client's ability to safely swallow, communicate effectively, and address cognition capacities. An individualized plan of care shall be documented, followed, and updated as needed on all patients who are appropriate for skilled Speech Therapy services. Treatment: Helps the client achieve goals set forth in the individualized plan of care by providing neuromuscular strengthening, coordination, and sensation to improve speech, voice, and swallow function, development of cognitive-linguistic skills, development of language skills for functional communication, and respiratory strengthening and coordination. Documentation: Ensures documentation of speech therapy services provided or attempted is accurate, comprehensive, timely, and in accordance with ASHA and TJC's standards of practice. Evaluations are written to include all required components and will be sent for certification timely. Treatment notes are written for each treatment given and accurately reflect both the treatment received as well as the length of the treatment. Progress reports are written at appropriate intervals throughout the patient's episode of care. Treatment outcome goals as well as level of progress towards goals is readily apparent in documentation. A discharge summary is written for each patient discharged. Supervision: Supervises Speech Language Pathologists in the Clinical Fellowship Year, therapy aides, and Speech Language Pathology students in accordance with standards set by ASHA, the state of employment, and by the facility. Contribution to healthcare team: Takes an active role in interprofessional relations through effective documentation in the medical record, actively participating and/or initiating care team discussions related to patient care, and educating patients, families, and other healthcare team members. Maintenance of the speech therapy department's integrity: Maintains professional expertise through continued education as necessitated by licensure and as appropriate for provision of specific treatments in accordance to current best practice. Assists the Rehabilitation Director in planning and coordinating the program and activities: Assists with the development of the outpatient therapy services including planning, development and marketing, staff education and training, foster professional relationships with MMC and community healthcare providers, work with director and growth team for program growth plan, coordinate with scheduling department for patient appointments, and oversight of daily operations of the outpatient clinic. Minimum Qualifications Minimum Education: Graduate from a Master's in Speech Language Pathology Program.  Minimum Experience: Two years experience in an outpatient setting.  Certification/Licensure: Certified as ASHA in Speech Language Pathology. Licensed in the State of New Mexico as a Speech Therapist. CPR required. Certification or completion courses must meet the standards of Memorial Medical Center.   EEOC Statement
