We are seeking a compassionate and dedicated Nurse Practitioner to join our interdisciplinary team, where you'll play a pivotal role in providing clinical care to our patients. As the lead member of our care team, you'll work alongside a dietitian, exercise trainer, and other clinicians, guiding patients through our comprehensive approach to health improvement.
Major Areas of Responsibility:
Introduce patients to our holistic philosophy and guide them through a personalized pathway to better health.
Collaborate with patients to establish achievable goals for weight reduction and overall health enhancement, including target body mass index.
Conduct thorough assessments, including vital signs, body composition, and comprehensive medical history reviews.
Work closely with our multidisciplinary team to interpret laboratory data and develop tailored care plans.
Prescribe pharmacotherapy, when necessary, as part of our comprehensive approach to treating obesity.
Provide ongoing support and counseling to patients, empowering them to make sustainable lifestyle changes.
Utilize our range of services, including our app and remote monitoring, to enhance patient care.
Ensure accurate and timely documentation of patient encounters and services provided.
Maintain open communication with referring providers to facilitate seamless care transitions.
Other Responsibilities:
Educate patients on our team-based approach to health improvement and the value of each component of our program.
Document all aspects of office visits promptly and thoroughly.
Collaborate with colleagues and administrative staff to deliver exceptional patient care.
Continuously adapt to evolving medical protocols and policies.
Education:
Minimum of a Master's Degree in Nursing from an accredited institution.
Work Experience:
Experience in evaluating and managing chronic medical conditions, particularly those related to obesity.
Licensure/Certification/Affiliation:
Licensure as a Registered Nurse and Nurse Practitioner in the state of practice.
Certification as a Nurse Practitioner by the AANP or ANCC.
Current CPR certification.
Skills, Knowledge, and Abilities:

Comfortable with medical decision-making and prescribing medication.
