JOB SUMMARY: As senior clinician on-site, manages acute and chronic care patients through preventive and primary care services in a fixed clinic (Orangeburg), through telehealth, and mobile health unit. Helps educate rural health professionals and supports clinical research.
Performs other duties as assigned.JOB DUTIES: 70% - Essential - Clinical Practice:
Delivers clinical care to patients in a team-based, patient-focused environment, via in-person and telehealth appointments; appropriately documents within the electronic health record for continuity of care and billing purposes; collaborates with nursing and other clinic staff; supports disease prevention, wellness, and chronic disease management; adheres to regulatory and compliance standards; serves as clinical preceptor for undergraduate and graduate students within his/her scope of practice; leadership on quality improvement projects to enhance the patient experience, patient safety and improve clinic efficiencies.
15% - Essential - Sponsored Programs Support:
Leads (when applicable) and assists with the development and implementation of clinical research and sponsored activity proposals; Provides direction for incorporation of research and sponsored activities into clinical workflow at fixed and mobile facilities, including CRH investigator-led and external collaborator activities; Contributes to development of study and sponsored activity protocols to ensure regulatory and reporting requirements are met; oversees majority of clinical support for research activities.
10% - Essential - Quality and Patient Safety:
Identifies quality risks and issues and creates appropriate corrective action plans; integrates quality improvement activities into clinician workflow to enhance patient experience and safety; Works with Quality Manager on accreditation, infection control, and other standards.
5% - Essential - Collaboration:
Assists the practice and CRH by developing and maintaining professional relationships with community partners regionally and across South Carolina and fill gaps in care and health outcomes; participates in community development and public health activities, including advisory boards and other groups as appropriate; serves as liaison with other clinical entities internally and externally to the university. MINIMUM REQUIREMENTS: Graduation from an accredited school of nursing with clinical experience.
Positions in this classification require licensure as Registered Nurse and hold official recognition as a Nurse Practitioner from the South Carolina Board of Nursing. Positions may be licensed to practice in the extended role and authorized for prescriptive authority.
Certified Diabetes Educator (CDCES) or substantial experience with diabetes and willingness to earn within 12-18 months from hire. SC license to practice as an Advanced Practice Registered Nurse. DEA certification for controlled substances. Valid SC driver's license. Certified in Basic Life Support.
PREFERRED REQUIREMENTS: Education - 1st Master's Degree - Graduation from an accredited school of nursing at the master's level or doctoral level
2+ years of clinical experience as an advanced practice registered nurse.
Work Exp 3+ years overall
Doctorate in Nursing Practice, Advanced education in Diabetes Management.
Emphasis on chronic disease management (diabetes, hypertension, obesity)
1+ years delivering health care services in rural and under-served areas. Experience in managing chronic disease patients. Use of an electronic health record and other technology in practice. Experience with clinical trials.

COMPENSATION INFORMATION Band: 09 (expected range: $ 81,388.00 - $ 120,000)


Salary is dependent upon several factors including, but not limited to, a candidate's previous experience, knowledge, skills and performance in accordance with Clemson's compensation guidelines .
