The Registered Nurse (RN) assigned to the VAPORHCS Patient Aligned Care Team (PACT) Primary Care Clinic functions as the PACT Nurse Care Manager (NCM) and may be assigned to the clinics located in Portland, Vancouver, Camp Rilea, Bend, Salem, Fairview, The Dalles, West Linn, Community Resource Referral Center (CRRC), or Hillsboro. *** This is NOT a virtual/remote position. *** Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: 5 years of RN experience, with at least 1 year of acute or ambulatory care within the last 5 years Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Moderate lifting, and carrying (15-44 lbs); Straight pulling (0.5 hour); Pushing (0.5 hour); Reaching above shoulder; Use of fingers; Both hands required; Walking, and Standing (4 hours); Kneeling (0.5 hour); Ability for rapid mental and muscular coordination simultaneously; Depth perception; Ability to distinguish basic colors; Hearing (aid may be permitted); Mental & emotional stability. ["The PACT NCM is a member of the PACT Teamlet (Primary Care Provider, Nurse Care Manager, Licensed Practical Nurse, Health Technician, and Clerical Associate) and supports the work of the expanded team (Pharmacist, Social Worker, Home Telehealth RN, Psychologist, and Dietician). Identifies and provides case management for high-risk cohort of patients utilizing chronic disease management data mining tools to include but not limited to Care Management Tool (CMT), Business Intelligent Strategic Learning (BISL), VISN 20 Access Report, VISTA, and VHA Support Service Center (VSSC) reports such as PC Almanac, Compass, Primary Care Management Module (PCMM) Care Needs Assessment (CNA). Utilizes other registries to manage disease processes such as Diabetic, Mammography, Cardiovascular, and Opioid Registries to access data base for pain management. Performs timely Post-Discharge/ ED/ Urgent Care and Walk-ins' using appropriate documentation templates/notes, codes, education. Assesses and enrolls eligible patients into nontraditional modalities of care such as Home telehealth and Telemedicine. Utilizes Teach and Motivational Interviewing techniques to encourage positive health behavioral change and Health Promotion and Disease Prevention. Provides patient and family education with a focus on self-management, prevention, and wellness, based on 'what matters most' to the patient. Reviews, tracks, and devises strategies to improve Healthcare Effective Data Information Sets (HEDIS) metrics, clinical reminders, panel access, timely encounter closures, and other performance metrics. Facilitates continuity of care by partnering with multidisciplinary services, internal and external to the Department of Veterans Affairs. Initiates and participates in various quality and process improvement activities that result in improved patient outcomes. Provide VA Video Connect (VVC), Clinical Video Technology (CVT), or Telemedicine. Responsible for maintaining knowledge of and skill in applying an intensive body of rules, procedures, and operations in telehealth related technologies and a variety of computer hardware/ software used in technology, data entry, retrieval, record maintenance, document, and report generation. May be required to perform specimen collection of (urine, sputum, blood, or stool) ensuring to utilize proper labeling, packaging, and transport instructions from the Primary Care Community Based Clinics or assigned areas to the main facility laboratory. Utilizes proper facility required patient identified methods and ensure patient confidentiality is maintained. Access and complete specimen orders from the hospital computerized system, utilize centrifugation (id required), and prepare specimen prior to laboratory testing. Exercises effective communication skills prior to, during, and after specimen collection, ensuring to review pertinent information with the patient. Maintains specimen integrity by using aseptic techniques, following department procedures, and observing and adhering to personal protective equipment policies and guidelines. Adheres to applicable infection control guidelines of hand hygiene before and after venipuncture of all patients. Incumbent will demonstrate knowledge, skill, and competency in venipuncture and specimen collection. Incumbent will be able to resolve order discrepancies by contacting the order originator for further clarification, notifying supervisor of unresolved orders. Must maintain a safe and secure working environment by adhering to local medical center standards, policies, and procedures as well as other federal or state regulating bodies to include but not limited to Joint Commission on the Accreditation of Healthcare Organizations (JCAHO), Occupational Health and Safety Administrator (OSHA), and the Center for Disease Control (CDC).
