Responsibilities
• Develop educational programs and materials to increase patient and family knowledge.• Assist patients in setting care goals and optimizing their health outcomes.• Provide continuing education to care providers and patients.• Develop care pathways based on evidence-based practices for optimal patient outcomes.• Ensure compliance with CMS requirements and standards.• Support an integrated care delivery model and collaboration between medical and nursing staff.• Follow up and resolve physician/patient issues and concerns.• Maintain knowledge of new clinical technologies and changes in care delivery models.• Perform onsite visits, assessments, and serve as a liaison between providers, hospital, and leadership.
Requirements
• BSN degree and RN license in Kentucky.• BLS/CPR certification within 5 days.• 3-5 years of RN experience in a clinical or hospital setting.
