POSITION SUMMARY/RESPONSIBILITIES As part of disease prevention initiative, this position will serve as the liaison between patients, clinics and providers in order to increase screening, vaccination, and linkage to care. This position will assist and guide patients through prevention services to include outreach and screening services, while providing education, support, and patient care coordination. This includes ensuring appropriate information is disseminated, patient is navigated to medical appointments, and follow-up with health care needs. All functions and behaviors are applicable to the clinical and community settings. Additional activities will include patient tracking and vetting, data entry, reporting, educational presentations, and recruit and facilitate vaccination process with pharmacy.
EDUCATION/EXPERIENCE Bachelor's degree in social sciences, health education or related field of study is preferred. Four years of experience in the area of health education, public health, social work and/or case management may be considered in lieu of the Bachelor's degree. Community Health Worker certification preferred. Knowledge and experience working with programs of similar scope and focus preferred. English/Spanish fluency is strongly desired.
