Job summary
The Masters Social Worker is responsible for performing social work assessments and interventions for hospitalized and emergency department patients.Responsibilities include crisis intervention, patient/family intervention, high-risk screening, brief counseling, referrals for financial or resource needs, and more.The role also involves arranging post-acute placement and engaging appropriate agencies or community resources for high-risk patients.

Responsibilities
• Providing developmentally appropriate care for all populations served• Advocacy and education for patient/family self-care management• Psychosocial management including crisis intervention and counseling support• Coordination of interdisciplinary care communication and continuity/transitions of care planning• Management and coordination of transition planning for psychosocially complex cases• Community resource coordination and expert consultation on health care resource management• Performance and outcomes management, including adherence to regulatory and accreditation requirements• Provide support and social work services to outpatients if directed• Participate in performance improvement teams and programs• Demonstrate behavior aligned with the mission and core values of the organization
Requirements
• Master's degree from a school of social work accredited by the Council of Social Work Education• 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting• Preferred minimum 3-Year Post-MSW healthcare experience• Required licensure and certifications: Licensed Advanced Social Worker (LASW:WA) OR Licensed Social Worker Advanced Associate (SWORKADVLI:WA) OR Licensed Social Worker Associate Independent Clinical (SWAIC:WA) OR Agency Affiliate Counselor (AAC:WA)
