John Knox Village Manager of Social Services John Knox Village is looking for a Manager of Social Services to Provide residents and families with a knowledgeable, resourceful counselor and advocate as they navigate complex transitions. Serve as liaison between residents/families and JKV Team members to ensure community goals are achieved.
The qualified candidate will be responsible for planning and administering social service programs. Supervise independent living Social Workers and Resident Relations Administrative Assistant. Assist in the development of policies regarding participation in facility planning for health and welfare services. Manage the Spiritual Life services, including spiritual counseling, support, and guidance to residents and their family members.
Responsibilities will include:
Manage, coach, counsel, and mentor Social Services in independent living and Spiritual Life Teams. Accurately identify at-risk residents and establish appropriate action plans producing resident and community satisfaction at 95% of the time. Counsel residents struggling with interpersonal losses, losses of functioning, or losses of position/status. Make appropriate referrals to mental health counseling or other mental health resources for residents as appropriate. Provide education to residents and families about resources available to promote maintaining independence. Provide education to residents and families about the transition process to higher levels of care, terms of life care contract and departmental roles. Present relevant information at the New Resident Orientation. Serve as a knowledgeable resource for residents suffering from dementia and provide education about the disease process, support groups, interventions, and programs to residents and families. Make referrals to home health for case management, nursing, aide, or companion, services as appropriate . Collaborate with the nursing department, VP of Health Care Services, and/or assisted living administrator 100% of the time in the assessment process for residents needing higher levels of care. Provide tours of Assisted Living and Skilled Nursing Facility buildings to residents and families. Facilitate smooth resident transition to Assisted Living or Skilled Nursing Facility levels of care, producing resident and community satisfaction 95% of the time. Recognize any volatile situation and act with appropriate sense of urgency, alerting the appropriate resources to the circumstances to validate the action steps needed to produce the desired outcome. Coordinate services across departments to resolve resident issues. Facilitate weekly Transitions meetings and relay pertinent information discussed 100% of the time to the proper departments. Refer residents to support services to address specific needs on or off campus, including bereavement or other appropriate resources. Manage Spiritual Life Team and their directives, including participating in annual Loss of Loved One services. Provide detailed, accurate information and support to families of deceased residents to assist in closing out the estate. Accountable for monitoring the process of vacating independent living resident units within 30 days of the transition. Communicate with appropriate parties. Obtain proper documentation. Maintain current knowledge of Medicare, supplemental, prescription, and long-term care insurance. Educate and advocate for residents in these areas, including the implications of choosing a Medicare Advantage plan with regard to the healthcare benefits at JKV. Facilitate enrollment in beneficial Medicare Part D prescription drug plans per resident choice at appropriate times under Medicare guidelines. The Required Qualifications are:
Master's Degree in Human Service related field (i.e., Social Work, Counseling, Psychology). Licensed Clinical Social Worker preferred. 5+ years of experience working with geriatric population. Clear understanding of all psycho-social aspects of the aging process. Medicare knowledge is a plus. Sharp assessment skills and excellent problem-solving skills. Ability to manage multiple tasks and prioritize effectively. Ability to accurately evaluate outcomes. Working knowledge of computers.
