The Role and Responsibilities:
We are seeking licensed practical nurses to join us in partnering with a leading national company supporting remote care coordination for Chronic Care Management (CCM) and other clinical needs across the country.
As an LPN, you will collaborate closely with our team and partners to ensure seamless care and effective communication. Your responsibilities will include, but are not limited to:
Conducting chart audits and reviews, updating patient records with precision using designated software.
Building strong virtual relationships with patients, caregivers, and team members to ensure exceptional experiences and outcomes.
Completing assigned audits and documentation accurately and efficiently.
Maintaining meticulous time tracking and documentation of care episodes.
Identifying and promptly communicating any concerns to relevant parties for timely follow-up.
Participating in required technical and clinical training within specified timeframes.
Following escalation protocols for urgent scenarios during calls.
Upholding patient privacy and strictly adhering to HIPAA compliance standards in the virtual care environment.
Maintaining an active and unencumbered nursing license in the relevant state(s) of care.
Availability to work a minimum of 25 hours per week, with flexibility for additional hours as needed.
Potential for evening and weekend shifts, with core hours likely Monday to Friday, 8:00 am to 7:00 pm in Central and Eastern Time Zones.
Qualifications:
Education: Graduate of an accredited degree program.
Experience: Minimum two years of nursing experience post-graduation.
Licensure: Active and valid compact LPN license.
Computer Proficiency: Basic computer skills required; ability to navigate virtual desktops and learn new software systems.
Communication: Effective interpersonal, oral, and written communication skills; ability to initiate communication regularly with cohort leads.
Language: Proficiency in English for phone, video conferencing, and written communication.
Professionalism: Courteous, empathetic, and clear communication, even in stressful situations; commitment to chosen work schedule and productivity in a remote environment.
Patient-Centered: Demonstrates patient-valued behaviors, respects patient needs, rights, and confidentiality.
Adaptability: Quick to learn new systems and workflows, comfortable in a virtual care startup environment.
Mission and team-driven: Enthusiastic about working with a supportive, compassionate, and collaborative team to make a lasting impact on patients, caregivers, and nurses using innovative technology.
Technical Requirements:
Dedicated, distraction-free room for HIPAA-sensitive video calls.
Fast and stable internet connection (at least 50 Mbps).
Laptop or desktop computer (Chromebooks not supported).
Additional computer monitor required.
