LCSW Care Coordinator Full Time Days ** Pay rate starting at $79,710/year; based on experience ** United Methodist Communities in Camden County, a 5-Star full-service senior living community that is a model in person-centered care community, located in beautiful Collingswood, NJ is seeking an LCSW Care Coordinator. Requirements for an LCSW Care Coordinator: Current NJ LCSW OR a Bachelor's degree in a related field from an accredited college or University. Case manager certification preferred. One year's experience in case management marketing; Two years of experience in case management marketing preferred. Knowledge of Microsoft Office. Responsibilities for an LCSW Care Coordinator: Gathers and evaluates all referral information and reviews information received and/or assesses potential residents for admission. Makes admission decisions and/or works with nursing on admission decision for healthcare. Builds and maintains referral network, conducting onsite visits as needed. Facilitate the admission process into healthcare and ensure paperwork is signed and processed in a timely manner. Verify Medicare coverage and/or other benefits. Participates in care conferences and other meetings as required. Follows residents to home post discharge to ensure they are okay; completes medication reconciliation; psychosocial assessment. Involved with post-acute discharge at home. Ensures departmental reports are completed in a timely manner.
