Innovation Coordinator works under the guidance of the State Director Innovative Services with the focus of Coordinator is to support people to live more effectively and meaningful lives by supporting them to secure nontraditional services that encompass a combination of staff supports and enabling technologies. This includes, residential, employment, and community supports. Key areas include, assessment, person centered / MAP's planning, linking people to services, training, and advocacy. The Charles Lea Center is a CQL accredited organization that is based in Spartanburg South Carolina and provides services in South Carolina, and North Carolina and recently was awarded a contract with the Tennessee Department If intellectual and Developmental Disabilities to provide services in their State. QUALIFICATIONS At least 18 years of age Ability to pass a background check upon hire Valid driver's license REQUIREMENTS Live in Tennessee area, preferably in Fort Campbell Fully Remote Bachelor's Degree in Human Services or a related field with t wo (2) years of experience in providing or coordinating services to people with disabilities and special needs and/or with case management services
-OR- Four (4) Years Case Management experience in providing or coordinating services to people with disabilities and special needs and/or with case management services.

A DAY AS THE INNOVATION COORDINATOR: As the Innovation Coordinator, you provide services and supports focused on achieving goals for the people we support. Provide technical assistance to Community Providers implementing services for people referred Advocate on behalf of person / guardian to assure that services being provided reflect the desires of person guardian. Upon successfully transitioning to community services conduct 30, 60, 90 day follow up to assure satisfaction with services and those desired outcomes are being achieved.
