Details of the offer

Care coordination for our Health System. Serve as the first point of contact with the Care Coordination Team for members or providers requesting assistance. Develop working partnerships with Community Health, Behavioral Health, Primary Health care providers regarding member needs and care plans. Develop basic care plans, contribute to and implement complex and integrated plans of care under the direction of clinical care coordination staff. Participate in multidisciplinary meetings to discuss and evaluate, develop, and update present care plan information as needed. Report member complaints to the appropriate authority for investigation and follow-up.

Requirements Education: HS Diploma. Preference to LPN/CMA/CNA. Minimum of 2 years' experience in a healthcare setting, outpatient primary care clinic, hospital, or health insurance plan. Will also be required to submit and pass a criminal background check with Federal, Tribal and State Law Enforcement.
