Position: Hospital Nurse Liaison



Pay : $68,000 - $78,000 / Salary Depending on experience


The Hospital Liaison enhances continuity of patient care by providing liaison between hospital and referral source staff. Works in a collaborative and credible manner with the hospital, nursing home, or other admissions staff, to help ensure they have a positive experience.

Responsibilities:

This individual would be the liaison to the assigned hospital(s). Market to all areas of the hospital including case management, oncology, ED and palliative care; making three to four contacts per day. Keeps practitioners and other hospital staff updated on patient status. Send daily monitoring regarding any GIP in that hospital to the case management department. Assist with Care Coordination. Participate in Pending meetings when available. PRN duties on a limited basis including but not limited to: Family contact call audits and Sup/Max audits LOS audits / ReCert Audits / TMIT Audits Coordinating ambulance transportation for patients who are discharging home. Obtaining information needed to be sent to place someone on the list for nursing home placement. Knowledge of all nursing centers Caris is contracted with (including GIP) in our service coverage area, and who is the point of contact for admissions. Explaining nursing home placement to families and that they will have to go to the nursing home centers prior to the patient arriving to complete admission paperwork for the nursing home. Knowledgeable of accepted insurances.
Qualifications:

Must be a Registered Nurse licensed in the state of operation. Must be knowledgeable regarding high quality hospice care and be able to personally provide patient care. Must be able to complete admissions paperwork properly, in accordance with company policy and hospice regulations, etc. Must possess good communication, organizational and interpersonal skills, be outgoing, perceptive and self motivated with the ability to work harmoniously with a wide variety of people.
