Hospice Coordinator Job Description: Dependable Hospice and Palliative Care's mission is to provide hospice services to our community through compassion and quality care. We work with a variety of communities to educate and carry-out the hospice philosophy by providing comfort care and elevating patients and their family's' quality of life towards end of life.
The Hospice Coordinator is primarily responsible for processing hospice referrals and works closely with the Clinical Manager and Admission Nurse to onboard patients onto service and determine hospice eligibility. As support for the intake team and clinical field staff, the Hospice Coordinator effectively ensures that proper procedures are maintained when onboarding new patients, as well as acts as the main point of contact between the clinical intake team and liaisons.
Qualifications: Collaborate with health professionals within and outside of the hospice team Ability to prioritize responsibilities and task switch effectively Strong aptitude for taking the initiative to work independently Ability to problem solve in a fast-paced environment Strong verbal and written communication skills Proficient in Windows OS and MS Office Suite Comprehensive understanding of Hospice Quality customer support and services Accurate data entry skills Detail-oriented Understanding of Hospice and Hospice philosophy Comprehensive understanding of HIPPA Rights Background in healthcare intake Preferred Qualifications: Understanding of Hospice and Hospice philosophy Comprehensive understanding of HIPPA rights Background in healthcare intake Responsibilities: Process hospice referrals by entering patient demographic, insurance information, and gathering medical records, progress notes, lab reports, and diagnostics for clinical review to determine hospice eligibility Arrange for authorization requests based on patient's insurance plan and eligibility Manage Medline supplies (i.e. briefs, wound care supplies, gloves, etc.) and order weekly supplies for patients by collaborating with RNs and CNAs on patient needs and supply utilization Support all administrative tasks with the Hospice program, while coordinating day-to-day operations Maintain accurate records of hospice admissions, deaths, discharges, and referrals on a daily basis Assist in durable medical equipment delivery/pick-up and patient transportation for same day hospital discharges for established patients and new admissions for the Admission RN and clinical intake team Compile and submit census reports on admissions, discharges, revocations, and deaths for weekly meetings with Hospice intake, Marketing team, President, Clinical Manager, and Hospice Administrator Organize medical records, weekly IDG rosters, admission books, hospice 101 binders.
