The Valeo Home Health team is looking to add a PRN Speech Language Pathologist for our patients in the Layton, Ogden and surrounding areas!Start of Care's and Discharge visit rates are $130-$140/PV.Valeo Home Health & Hospice is a 4 time Best of State and 3-time Top Workplace winner, who services Utah, Salt Lake, Weber, and Davis counties.

Responsibilities
• Performs accurate and comprehensive speech, language and swallowing assessments.• Obtains physician orders for certification and recertification of home speech therapy services.• Ensures that changes in patients' clinical status are communicated to other healthcare team members and HHA office staff verbally and in writing in a timely manner.• Ensures that physician orders are communicated to other healthcare team members and HHA staff in a timely, accurate and thorough manner.• Coordinates care planning and delivery with the physician and other healthcare team members and HHA staff to facilitate optimal patient outcomes.• Delivers speech therapy modalities appropriate to the patient's clinical status, needs and age.• Able to appropriately assess and reassess a patient's pain.• Utilizes appropriate pain management modalities that include, but are not limited to, physical therapy modalities.• Educates the patient and/or family in effective pain management modalities.• Provides speech therapy modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.• Supports and maintains a culture of safety and quality.• Educates the patient and/or family in effective pain management modalities.• Addresses and supports cultural practices providing such practices do not harm the patient or others, and do not interfere with the planned course of speech therapy.• Accurately completes OASIS data sets.• Complete patient documentation on time.• Attends in-service and staff meetings as needed.• Completes assigned training on time.• Completes annual education requirements.• Maintains patient confidentiality at all times.• Performs other duties as assigned.
Requirements
• Demonstrate knowledge and skills necessary to provide care to the geriatric and adult population.• Compassion and empathy for patients.• Attention to detail regarding a patient's problems to analyze what they need to ease their pain and to prevent further injury.• Interpersonal skills to build positive relationships with patients, families and staff.• Communication skills to interact clearly with doctors, family members, patients and the entire team of care.• Master's Graduate in the area of Speech Therapy or Speech Pathology from an accredited university or college.• Certificate of Clinical Competence in Speech-Language Pathology awarded by the American Speech Language and Hearing Association.• For Medicare Certified Agencies: Meets the education and experience requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech-Language-Hearing Association; or Meets the education requirements for certification and is in the process of accumulating the supervised experience required for certification• Current valid state Speech Therapy license.• Prior home health experience preferred.• One (1) years of work experience.• Prior home health experience, preferred.• Valid state driver's license and reliable automobile.• Proof of current automobile insurance.• Current TB testing.• Criminal background check.• Current CPR card.
