• Develop a plan of care based on physician orders, client input, and nursing assessment.• Educate clients and their family members according to their specific needs.• Train primary caregivers to ensure optimal functioning for each client.• Coordinate client care through documentation and communication with physicians and caregivers.• Provide nursing care in patients' homes using various skills such as phone triage and wound care.
Requirements
• Active RN License in the state of application.• Valid CPR certification.• Preferred: Medicare Skilled Nursing experience.• Basic understanding of Oasis.• 1-year RN experience in a healthcare setting.• Compliance with health-related requirements, including vaccinations and testing.
