As a Home Health Physical Therapist, you will work collaboratively with our care team to supervise and implement patient care plans. Your responsibilities will include conducting assessments, creating treatment plans, and providing hands-on physical therapy services to patients in their homes. You will have the opportunity to work one-on-one with patients, helping them regain mobility, reduce pain, and improve their overall quality of life.
This position is based in Paw Paw and requires driving to and from patient locations. Requirements Requirements: Graduation from a Physical Therapy program (Bachelor's or Master's degree) from an accredited college or university. A valid state-issued Physical Therapist license in the relevant state. Minimum of one year of experience in a healthcare setting. Valid driver's license and ability to travel to patient locations. Strong time management and organizational skills. Proficiency in utilizing tablets, mobile phones, and electronic medical record (EMR) software.
