A licensed practical nurse is a clinically competent position that provides care to a group of home health patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members to assure resident safety.
Perform initial data collection on all patients/residents and follows up on data collection as per policy. Participate in implementing person driven plans of care as developed by the Registered Nurse within established policies and procedures and continually works to meet individual patient/resident needs. Demonstrate knowledge of medications and their correct administration based on age of the patient/resident and their clinical condition. Observe, document and report patient/resident responses to medications. Follow patient/resident, family and caregiver education plan of care as developed by the Registered Nurse, clearly communicates information and documents patient/resident response to education. Document according to current standards and policies. Plan for patient/resident discharge, collaborates with the Registered Nurse and discharge planners for long and short term patient/resident needs. Coordinate and direct patient/resident care appropriately. Perform charge nurse or building responsibilities when assigned. Utilize the principles of Relationship Based Care while performing duties. Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company. Performs other duties, tasks and/or projects as assigned. QUALIFICATIONS REQUIRED
Current state licensure as a Practical Nurse, which must be in good standing BLS certified Valid Class A Driver's License QUALIFICATIONS PREFERRED
