The Health Care Navigator plays a pivotal role in providing essential support to veterans and their families, ensuring access to health care services and fostering self-directed recovery. This position requires a strong background in the human services field, along with excellent communication skills to effectively collaborate with various community partners and support systems. The navigator assists veterans in optimizing treatment interventions, accessing community services, and adhering to health care plans while upholding Chestnut's culture of customer service excellence.
Complete required data entry, updating SSVF databases, and paperwork in accordance with program and Veteran Administration policies and procedures, including health care assessments, treatment plans, and progress notes. Provide veteran-driven services identified in the Health Care Plan, including connecting veterans to community services, case management, and linking to additional resources. Act as a health coach, supporting veterans in optimizing treatment interventions, promoting self-care, and providing health education services. Promote the recovery model of treatment by assisting veterans in regaining control over their lives through self-directed recovery and utilizing recovery language in documentation. Foster positive interactions with the VA, veterans' medical staff, and veteran support organizations to sustain a favorable program reputation. Provide consultation and care collaboration with the VA and veterans' primary care providers. Serve as a resource for education and support for veterans and families. Attend meetings, participate in program development activities, and provide support to other team members as necessary. Promote Chestnut's culture of customer service excellence through adherence to behavioral standards. Assume leadership roles, assist in training new staff, and serve as a mentor for other SSVF team members. Maintain confidentiality of organizational information gained in the course of performing job responsibilities. Perform other duties as assigned by the supervisor. Candidates for this role should possess a minimum of a high school diploma with 20 years of experience in the human services field, military service, or related administrative area. Alternatively, a Bachelor's degree in a helping profession or related administrative area with 10 years of relevant experience, or a Master's degree with 5 years of experience, is required. Preference is given to military veterans or immediate family members of veterans. Strong telephone skills, professional communication abilities, and proficiency in Microsoft Office tools and electronic health records are essential. A valid driver's license, private automobile insurance, and basic computer skills are necessary for this position.
