Responsibilities
• Provides continuity of care between care settings• Develops relationships with patients and multidisciplinary team• Manages high-risk, complex patient care• Coordinates patient's care plan communication with physicians• Prepares and reinforces post-discharge care plan• Identifies barriers to care and improves processes• Collaborates with leadership to improve the clinical experience• Adapts behavior to specific patient population• Performs other related duties as required
Requirements
• Registered nurse diploma• 2 years of nursing experience• Current registered nurse (RN) license• Basic Life Support (BLS) certification• Proficiency in using computers and software• Effective verbal and written communication skills• Excellent conflict resolution skills• Ability to work flexible schedule and travel• Associate's or bachelor's degree in nursing preferred• Certification in clinical specialty area preferred
