Full Time Home Health Patient Scheduling Coordinator

Monday through Friday, 8:00 AM to 5:00 PM Overview:
The Scheduling Coordinator is responsible for the coordination of patient/client physician orders, pre- authorizations and patient/client visits of physician orders, pre-authorizations and patient/client visits. Accurate and timely communication of scheduling changes between office and staff. Responsible for matching patient needs with staff talents and knowledge. Please apply on line for this position here: RESPONSIBILITIES Facilitates communications and works effectively between staff, patient/families, vendors, providers Completes scheduled work flow by assigning appropriate team members to planned frequency of visits Assist agency personnel in the care coordination of patient/client services Serves as a liaison between the organization, referral services, patients, clients and personnel Assists in identifying opportunities for additional/improved services and methods Actively participates in IDT meetings, department meetings, in-service, QAPI activities, and other related activities as appropriate Consistently represents Signature Healthcare at Home with integrity and professionalism to all internal and external customers Adheres to HIPAA regulations per Company policy and Federal regulations Performs other duties and activities as delegated by the Administrator/Clinical Manager QUALIFICATIONS High School diploma or College degree preferred Minimum two years' experience in administrative position in healthcare setting Minimum one-year experience in scheduling experience in healthcare setting Knowledge of medical terminology Demonstrates good communications, flexibility and cooperation in performing job responsibilities Possess a valid driver's license and insurance in accordance with state and/or organization requirements Basic computer skills and knowledge of office technology Please apply on line for this position here: or Contact: Sheena Saijo Regional Talent Acquisition Specialist Cell: Fax: Email:
