Responsibilities
• Provide training to clients in daily living skills• Facilitate development and/or enhancement of personal support systems within the client's community• Assist clients in creating and participating in recreational activities• Perform bimonthly apartment inspections and provide input/skills training on household management• Maintain work productivity standards and document according to policies and procedures• Complete all required training and work cooperatively with other departments• Participate in all required meetings as determined by supervisor• Other duties as assigned
Requirements
• HS Diploma/HSE/GED is required• Two years of mental health or direct health care experience preferred• Reliable transportation, valid driver's license, and proof of continuous auto insurance required• Must be able to work any day of the week Monday-Sunday 8am-8pm• Must be flexible on schedule• Position requires travel across multiple counties• Mileage will be reimbursed
